*Full voting members with more than one cemetery have two options: Option 1: Register as one company, combining the total number of interments. The combined list of cemeteries will have one vote. Combine interments from all cemeteries to determine amount to pay toward the Educational Fund. List all cemeteries and their contact information on a separate sheet and attach it to this form. Option 2: Each cemetery has their own membership and vote. Fill out a membership dues form for each cemetery.
