*Full voting members with more than one cemetery have two options: Option 1: Register as one company, combining the total number of interments. The combined list of cemeteries will have one vote. Combine interments from all cemeteries to determine amount to pay toward the Educational Fund. List all cemeteries and their contact information on a separate sheet and attach it to this form. Option 2: Each cemetery has their own membership and vote. Fill out a membership dues form for each cemetery.
Active Membership: - Full voting member (owner' managers, or their designee) - $75.00 plus fees per number of interments, entombments, or inurnment s made in the preceding year, regardless of the nature of remains or the age at time of death.
